Instructions for Payment By Your Insurance
STEP #1- Start by filling out the MEMBER FORMS on the left. If the insurance policy is in your name, That's all we need.
STEP #2 - If the insurance policy is in SOMEONE ELSE'S NAME, that person (called the Primary Policy Holder) must ALSO complete the PRIMARY POLICY HOLDER forms.
WHEN DO YOU NEED ALSO NEED THE DURABLE POWER OF ATTORNEY?
Answer: We will let you know if your insurance company requires this form.
Some insurance companies will make the checks payable to YOUR NAME instead of Recovery Help. We cannot deposit them and so your bill remains unpaid!
The Durable Power of Attorney form is giving us very limited legal power. You are ONLY giving us permission to endorse & deposit the checks payable in your name.
It does not allow us to act in your behalf for any other legal matters.
If the insurance policy is in someone else's name, you BOTH need to sign a durable power of attorney.
